Client - We are asking each of our clients to use our online form to share current household information so we may update our records.

 

Filling out this form will ensure we have the most current information on hand for your family. Family size, earnings information, and requirements may change from time to time, and it is important for us to have accurate information on file. 

 

If for any reason you are unable to fill out the form online, we are happy to provide you with a paper copy that you may return to the South Palo Alto Food Closet on your next visit. You will have until the end of December to respond either online or via a paper copy.
